The Root Cause of Acid Reflux: Why It's Rarely Too Much Acid

The burning sensation of acid reflux is one of the most common digestive complaints, and conventional medicine almost always treats it by turning off your stomach acid. While acid suppressing medications might bring temporary relief, they completely ignore the chemical environment of your gut. In functional medicine, we recognize that reflux is rarely caused by an overproduction of acid. Instead, it is frequently driven by a chemical breakdown in digestion, specifically low stomach acid, poor bile flow, and microbial dysbiosis.

To understand why this happens, we have to look at the role of stomach acid. Your stomach is supposed to be a highly acidic environment. Adequate hydrochloric acid is essential to break down proteins, absorb minerals, and defend against foreign microbes. When you actually have low stomach acid, digestion slows down entirely. This lack of acidity prevents your food from breaking down efficiently and directly contributes to bloating, reflux, bacterial overgrowth, and nutrient deficiency.

This lack of acid also disrupts downstream chemical signaling. Your pancreas and gallbladder rely on the acidity of your stomach contents to trigger the release of digestive enzymes and bile. Bile is absolutely essential for fat digestion and microbial regulation. When poor bile flow joins low stomach acid, your food remains largely undigested as it moves into the small intestine. This creates the perfect feeding ground for opportunistic bacteria, leading to a state of imbalance known as dysbiosis.

Dysbiosis and the resulting fermentation are major chemical drivers of reflux. When microbes metabolize undigested carbohydrates and fibers, they produce significant amounts of gas. This fermentation creates excessive upward gas pressure that physically forces the lower esophageal sphincter open. The trace amounts of acid that escape into your esophagus are enough to cause severe burning, even if your overall acid production is actually too low. This is exactly how unresolved upper digestive chemical issues eventually trigger the lower intestinal fermentation and hidden gut imbalances that drive hormonal skin issues and bloating.

Relying on daily acid blockers only worsens this chemical cascade. By artificially suppressing what little acid you have, these medications further impair digestion, invite more bacterial overgrowth, and reduce your ability to absorb critical nutrients like B12 and iron. Resolving your symptoms requires restoring your natural digestive chemistry, not suppressing it.

A root cause approach means testing rather than guessing. We look at the complete picture of your digestion, evaluating enzyme production, bile flow, and the presence of dysbiosis or infections like H. pylori. Just as standard medical panels often miss the early signs of insulin resistance, standard reflux care completely misses these underlying chemical drivers.
